I have a gourami with a white sore in top of his head. I think it might be hole in the head disease. It's usually white but sometimes it is a light pink. It has gotten larger over the past few days. I have been trying to put him in an isolation tank for treatment and to isolate him but he is still quite spunky... the disease, whatever it is, it's not causing him to 'act' sick.

My first question is how should I go about treating it? Secondly, is it contagious? I have heard that this disease usually doesn't happen when you have a bio-wheel... which I have had for about two months. No other fish in my tank have had HITH disease and I have had all the fish in there for quite a while (maybe oct-nov). The last disease I treated (a few weeks ago) was an out break of Ich, which has since disappeared. Could the Ich outbreak have something to do with the gourami's problem?

It sounds more like a bacterial viruse or infection. A viruse called Lymphocystis has greasy looking buff colered shiny white patching. A good gravel vac with a 20% water change and two tablespoons of salt will clear it up in a week or so, if its the viruse... Pink to red spoting could be Costia. QuickCure/Rid-Ich, or any other formaldehyde/malachite green medication will clear it up...I would do the gravel vac and water change, treat with both salt and QuickCure, and feed a medicated food for 10 days.......By the way, HITH disease starts out with pin sized holes near nostrils, gill plates, or near eyes and gradually increse in size untill large portions of the cranial is destroyed. Said to be a vitamin and/or mineral deficiency. Feeding red earthworms seems to be a good treatment in most cases.........Frank
